Relevance of the Research

Combined post-burn cicatricial strictures of the pharynx, esophagus, and stomach represent a complex medical problem requiring a comprehensive diagnostic approach and individualized surgical management. These injuries may lead to severe impairment of gastrointestinal tract patency and significantly affect patients’ quality of life.

Therefore, improving diagnostic methods, developing new reconstructive and plastic surgical techniques, and implementing a staged system of specialized surgical care are of considerable practical importance.

Aim of the Research

To develop new reconstructive and plastic surgical techniques and implement a staged system of specialized surgical care in order to improve treatment outcomes in patients with combined post-burn cicatricial strictures of the pharynx and esophagus.

Materials and Methods

The study included an analysis of the examination and treatment outcomes of 398 patients treated between 2000 and 2023.

The clinical research base was the Republican Specialized Scientific and Practical Medical Center of Surgery named after Academician V. Vakhidov.

The research employed clinical and medical-history assessment, radiological and endoscopic examinations, bronchoscopy, multislice computed tomography (MSCT), as well as sociological and statistical methods of analysis.

Scientific Novelty

The study investigated the specific features of combined post-burn cicatricial lesions of the pharynx, esophagus, and stomach, while improving diagnostic approaches and the selection of surgical management strategies.

A new author-developed technique for constructing a pharyngocolonic anastomosis was developed to improve the effectiveness of reconstructive and restorative surgical procedures. The method was granted Patent of the Republic of Uzbekistan No. IAP 05835 on May 22, 2019.

Main Research Findings

The analysis of 398 patients revealed the following distribution of combined injuries:

  • Combined involvement of the esophagus and stomach — 62%;

  • Combined involvement of the pharynx, esophagus, and stomach — 21.6%;

  • Combined involvement of the pharynx and esophagus — 16.3%.

In 216 patients (54.3%), impaired esophageal patency was the predominant clinical manifestation.

A total of 68 bypass pharyngocoloplasty procedures were performed as part of the study. With the newly developed technique, the incidence of anastomotic leakage was 2.9%, compared with 7.4% in the comparison group (p<0.05).

The incidence of cicatricial stricture following the new technique was 5.1%, compared with 24.1% in the comparison group.

Practical Significance

The findings contribute to improving the diagnosis and surgical treatment of patients with combined post-burn cicatricial strictures of the pharynx, esophagus, and stomach.

The developed surgical technique is aimed at reducing the incidence of postoperative complications and improving the outcomes of reconstructive and restorative surgical procedures.

Implementation of Research Findings

The research findings were introduced into clinical practice in 2026 at the Khorezm branch of the Republican Scientific Center for Emergency Medical Care and within the healthcare system of Samarkand.

Publications and Research Dissemination

A total of 25 scientific publications have been produced on the dissertation topic, including 9 scientific articles: 7 published in national scientific journals and 2 in international scientific journals. In addition, 1 patent was obtained.

The main research findings were presented at 5 scientific and practical conferences, including 3 international and 2 national conferences.

Conclusion

The research findings are aimed at improving diagnostic approaches and surgical management in patients with combined post-burn cicatricial strictures of the pharynx, esophagus, and stomach. The developed reconstructive and restorative surgical technique, together with the implementation of a staged system of specialized care, has important practical significance for improving treatment outcomes in this patient population.
